Here is your Action Plan for Recovery
  • Arrive 15 minutes early or save time by filling out the First Visit forms below. Be sure to bring your forms and insurance card if applicable.

  • Bring all appropriate records for review, such as an MRI.

  • Dr. Thomas will dedicate an hour with you to discuss your case and treatment options.

  • If you are going to change or cancel an appointment, please call at least 24 hours before your appointment or you will be charged $50. Our schedule is centered around you and we want to be respectful of all our other patients who are waiting for an appointment.
